All Fayette County Community Youth Football players and cheerleaders are required  to have a complete physical performed prior to participation.  Please have a signature on the attached form from your physician prior to participation. Safety is our top priority and it starts with getting clearance form a professional before the start of the season.  

Remember to email (or just take a picture and email) the completed physical with the participant's full name and age in the file name to fayettecountyfootball@gmail.com
